Job summary

My Place Home For The Homeless is seeking a Case Manager to support permanent supportive housing and a temporary shelter program. The role is dayshift, with duties spanning assessing client needs, coordinating with networks, and linking clients to health, childcare, and social services.

Responsibilities include assigning social workers for crisis response, evaluating services for quality, and advocating for community needs while building collaborations with policymakers and local agencies.

Qualifications

• Bachelor’s degree in social work, psychology, sociology, healthcare or related fields; MSW preferred.
• Licensed or certified by state board is a plus.
• Proficient with Microsoft Office (Word, Excel, PowerPoint).
• Willing to work nights and weekends when required.
• Ability to travel locally for client meetings.

Responsibilities

• Assess client needs and develop strategies to help them.
• Coordinate with support networks and community resources.
• Refer clients to health and childcare resources.
• Assign social workers for crisis response.
• Advocate for clients and develop community programs.
