Job summary

Volunteers of America Colorado seeks a Housing Case Manager for the Lakewood Navigation Center. You will conduct needs-based assessments, determine eligibility, and provide individualized case management to support shelter stability and housing goals.

You will assist participants in securing housing, coordinating with landlords, and connecting them to income and public benefits. Strong organizational skills and a CO driver’s license are required.

Qualifications

• Bachelor’s degree in human services or related field.
• At least 1 year of experience with people experiencing homelessness.
• Strong organizational and planning skills; works well in a team.
• Colorado driver’s license and a reliable vehicle.
• Background checks and funder-required trainings within 30 days of hire.
• Preferred: familiarity with housing market information and crisis intervention.

Responsibilities

• Conduct needs-based assessments, determine program eligibility, and provide individualized case management.
• Assist participants in securing and maintaining housing and connecting to benefits.
• Coordinate outreach with service providers and multidisciplinary teams.
• Maintain records in Service Point and HMIS; ensure timely data entry and reporting.
• Develop and monitor quality housing-focused services; apply best practices.
