Qualifications

Responsibilities

• Uphold vision and goals for effective case management and CM program.
• Provide case management and support services to shelter guests.
• Meet with each guest within 72 hours of intake.
• Maintain knowledge of community resources and trainings.
• Monitor daily logs and incident reports for guest needs.
• Prioritize urgent needs while tracking ongoing needs.
• Identify mandated reporting issues and file reports.
• Attend daily staff huddles and scheduled CM meetings.
