Job summary

The Salvation Army St. Cloud Corps in Saint Cloud, MN is seeking a dedicated Case Manager to provide individualized, person-centered services to shelter participants and families experiencing homelessness.

You will assess needs, develop service plans, connect clients with housing, employment, health resources, and advocate to support housing stability and self-sufficiency, working as part of a supportive team.

Qualifications

• Bachelor's degree in social work, human services, psychology, sociology, or a related field preferred.
• Minnesota state licensure is required if the degree is in social work.
• One year of experience providing case management, social services, housing services, or related support services.
• Experience working with individuals and families experiencing homelessness or housing instability preferred.
• Must pass background checks, possess a valid driver's license, and meet MVR clearance standards.

Responsibilities

• Provide person-centered case management services to shelter participants and develop individualized service plans.
• Complete assessments, monitor progress, and address barriers to housing, income, employment, and health.
• Coordinate referrals and connections to community resources, landlords, and partner agencies.
• Offer coaching and education on housing stability, budgeting, employment readiness, and life skills.
• Participate in team meetings, case consultations, and program activities; support a safe environment.
