Qualifications

• Associate's degree in human services or related field required.
• 1 year of experience providing case management services focused on pre-tenancy and tenancy-sustaining services.

Responsibilities

• Establish monthly rehousing meetings with families and document outcomes.
• Review barriers to housing such as credit scores, criminal records, and payment history.
• Collect and organize required documents to maintain participant files and assist with obtaining missing items.
• Create individual housing plans and budgets to promote housing stability and self-sufficiency.
• Document all case management efforts in the agency system with timely notes and reports.
• Provide education about affordable housing options and refer participants to support services.
• Coordinate with stakeholders and participate in training and program development.
