Job summary

Help USA is seeking a Case Manager Supervisor for the Ascend program at Bronx, NY 10454.

You will provide leadership to the Case Managers at a temporary housing shelter, coordinating a comprehensive program of on-site and community-based services to help residents achieve outcomes, including assessment, counseling, service planning, and linkages to entitlement, mental health, substance abuse, employment, medical, education, childcare, and day care.

Qualifications

• Bachelor's degree in social work or equivalent experience.
• Minimum 2 years of case management, assessment, counseling, and crisis intervention.
• Supervisory experience in case management preferred.

Responsibilities

• Ensure quality services through team meetings, supervision, and case conferences.
• Identify service gaps and develop linkages.
• Review residents' complaints and ensure timely follow-up.
• Document staff training and ensure completion of required courses.
• Ensure program compliance with government regulations.
• Submit program statistics and reports accurately and timely.
