Lantern Community Services seeks an experienced Case Manager for its men’s shelter in Far Rockaway, Queens. You will assess clients, link them to services, plan actions toward stabilization and housing, monitor progress, and document contacts. Prior homeless/mental health experience is preferred.

The role involves flexible hours, potential evenings/weekends, and working within an interdisciplinary team to support clients’ independent living goals.

Qualificações

• High school diploma, HSE or GED required.
• Minimum 2 years of social services experience.
• Possession of a valid Food Handler’s License or ability to obtain within 6 months.
• Experience with homeless, mentally ill, substance abuse, and/or low-income populations.
• Strong relationship-building and computer skills (Word and Excel).

Responsabilidades

• Conduct intake and assess clients to orient them to the program and develop an service plan.
• Meet weekly with clients to help achieve individual service plan goals.
• Coordinate services with Housing Specialist and external partners.
• Advocate for clients around entitlements and services.
• Provide crisis intervention and supportive counseling to aid decision-making.
• Document case management activities per agency guidelines.
• Complete housing applications for clients based on needs and functioning.
• Aim to achieve monthly placement quotas as assigned.
• Facilitate workshops and escort clients as applicable.
• Attend staff meetings and trainings to enhance job-related skills.
• Conduct weekly or biweekly unit/dorm inspections.
