Lantern Community Services in Bronx, NY is seeking a dynamic Case Manager to support formerly homeless individuals and families in a supportive housing environment. You will help residents stabilize their lives and work toward housing stability and personal goals, with a focus on mental health, substance use, and medical needs.

The role requires solid clinical knowledge, strong teamwork with an interdisciplinary staff, and a proactive, solution-oriented approach.

Formación

• High school diploma, HSE or GED required.
• Minimum 2 years of social services experience.
• Food Handler’s License or ability to obtain within 6 months.
• Bilingual Spanish/English preferred but not required.

Responsabilidades

• Collaborate with residents to develop recovery-oriented service plans with goals.
• Provide integrated counseling and case management to promote housing stability.
• Use a harm reduction approach to support a diverse caseload around mental health and substance use.
• Make referrals and liaise with care coordination, health, mental health, and employment providers.
• Engage residents through regular visits, community activities, and appointment escorts.
• Develop safety plans and de-escalate crises in collaboration with the team.
• Maintain confidential records documenting interventions and interactions.
• Collaborate with Leasing and Compliance to ensure housing stability.
