Job summary

Lantern Community Services in the Bronx is seeking experienced Case Managers for its new women’s shelter to help residents stabilize housing goals. You will conduct intake, develop individualized service plans, link clients to housing and supportive services, monitor progress, and document all interactions.

The position supports a shelter housing 200 women and is based in the Bronx. This full-time, non-exempt role requires two years of social services experience and a high school diploma;

Qualifications

• High school diploma, HSE, or GED required.
• Minimum of two (2) years of social services experience.
• Valid Food Handler’s License or ability to obtain within six (6) months of hire.
• Experience with homelessness, mental health, substance use, or low-income populations.
• Strong relationship-building and communication skills.
• Strong computer skills, including Word and Excel.

Responsibilities

• Conduct intake and assessments to orient clients to the program and develop service plans.
• Meet weekly with clients to support progress toward goals.
• Coordinate services with Shelter staff, Housing Specialist, and partners.
• Advocate for benefits, entitlements, and service access.
• Provide crisis intervention and supportive counseling as needed.
• Document all case management activities per guidelines.
• Complete housing applications based on client needs and eligibility.
• Achieve monthly housing placement quotas; facilitate workshops and escort clients as needed.
• Attend staff meetings and trainings; work flexible evenings/weekends.
