Lantern Community Services is seeking experienced Case Managers for a brand-new women’s shelter in the Bronx. You will conduct intake, assessments, and develop individualized service plans to stabilize clients and support housing goals.

The role requires coordinating with shelter staff, the Housing Specialist, and external partners. Responsibilities include providing crisis intervention, documenting case management activities, advocating for benefits, and assisting with housing applications.

Qualifikationen

• High school diploma, HSE, or GED required.
• Minimum of two years of social services experience.
• Food Handler’s License or ability to obtain within six months.
• Experience with homelessness, mental illness, substance use, and/or low-income populations.
• Strong relationship-building and communication skills.
• Strong computer skills including Word and Excel.
• Ability to work evenings and/or weekends on a rotating schedule.
• Prior experience working in a homeless shelter preferred.

Aufgaben

• Conduct intake and assessments to orient clients to the program and develop individualized service plans based on needs and goals.
• Meet weekly with clients to support progress toward service plan goals.
• Coordinate services with the Housing Specialist, shelter staff, and external partners.
• Advocate for clients regarding benefits, entitlements, and service access.
• Provide crisis intervention and supportive counseling to strengthen clients’ decision-making and coping skills.
• Document all case management activities in accordance with agency and funding guidelines.
• Complete housing applications based on clients’ needs and eligibility.
• Achieve assigned monthly housing placement quotas; facilitate workshops and groups.
• Escort clients to appointments as needed.
• Conduct weekly or biweekly unit or dorm inspections.
• Attend staff meetings and trainings to enhance job-related skills.
• Work flexible schedules, including evening and weekend shifts as required.
• Perform other related duties as assigned.
