Qualifikationen

• Two years case work experience in a social service agency or fieldwork.
• SOAR experience/training preferred.
• Equivalent combination of education and experience accepted.

Aufgaben

• Conducts casework interviews with clients to obtain information for eligibility and case status.
• Identifies psychosocial, economic, and physical needs and plans appropriate services.
• Develops and implements a case plan with intervention strategies.
• Assists clients with housing search and placement activities.
• Refer clients to community resources and advocate for services.
• Prepares case notes, writes reports, and inputs data into HMIS.
• Reviews assigned cases to determine continuing need for services.
