Job summary

The Salvation Army in La Crosse County is seeking a Case Manager I to provide on-site shelter case management services to eligible clients. You will identify needs, coordinate services, and monitor progress with a focus on housing placement and community resources.

Education: Bachelor's degree in social work or related field preferred; experience of two years in social services or fieldwork is expected. SOAR experience preferred. Full-time role with competitive benefits.

Qualifications

• Bachelor’s in a social work related field preferred.
• Two years of case work experience in a social service agency or fieldwork.
• SOAR experience/training preferred.
• -Or any equivalent combination of education and experience that provides the requisite knowledge, skills and abilities.

Responsibilities

• Conducts casework interviews with clients to obtain information for eligibility and status.
• Identifies psychosocial, economic, and physical needs; assesses support systems and community resources.
• Develops and implements a case plan with intervention strategies.
• Assists client with housing search and placement activities.
• Refers clients to community resources and advocates for services.
• Prepares case notes, inputs data into HMIS, and writes reports.
• Reviews cases to determine continuing need for services and participates in consultations.
