Job summary

Qualifications

• High School Diploma or equivalent; Addiction certification preferred.
• Minimum 1 year experience with homelessness or addiction; ministerial counseling preferred.

Responsibilities

• Collaborate with residents to develop their initial case plan.
• Assist residents with paperwork for goals, referrals and documentation.
• Set and monitor goals across categories: ID, job, education, addiction treatment, spiritual growth, finances, legal, housing.
• Maintain objective case notes using SOAP; keep weekly calendar in the Client Management System.
• Meet weekly with residents to evaluate progress and stability; document interactions.
• Refer residents to agencies per case plan and assist with life skills classes.
• Ensure living areas comply with shelter guidelines and provide staff relief when needed.
• Conduct random drug screens and complete discharges with accurate information.
• Meet with supervisor regularly to report progress and utilize resources.
