Job summary

St. Matthew’s House (Southwest Florida) seeks a dedicated Case Manager to provide resident-focused support in Emergency Housing/Shelter. You will advocate, set goals, access community resources, and work one-on-one to track progress toward self-sufficiency.

The role emphasizes compassionate, faith-based care and collaboration with staff, volunteers, and community partners to help residents achieve stability and transition to sustainable housing.

Qualifications

• Minimum 1 year of experience with individuals who are homeless or suffering from addiction.
• High School Diploma required; Addiction certification preferred.
• Must be able to stand for 4 hours at a time and lift at least 25 pounds.
• Must be able to bend or kneel to the ground.

Responsibilities

• Collaborate with residents to develop initial individual case plans.
• Assist residents with paperwork for goals and referrals.
• Establishs goals in categories (ID, job, education, treatment, spiritual growth, finances, legal, housing).
• Prepares and maintains objective case notes using the SOAP method.
• Maintains a weekly calendar of all scheduled Case Management appointments.
• Meets weekly with residents to evaluate progress and establish stability.
• Refers residents to agencies based on need and case plan.
• Works with volunteers to provide life skills classes.
• Checks living areas for compliance with Shelter guidelines.
• Provides relief for staff during breaks.
• Performs random drug screens as needed.
• Completes discharge documentation with accurate information.
