Job summary

St. Matthew's House in Naples, FL is seeking a Case Manager to support residents in emergency housing/shelter through individualized planning and advocacy.

You will develop goals, help residents access community resources, document progress with SOAP notes, and coordinate with staff and volunteers to promote self-sufficiency and stable housing. Qualifications include a High School Diploma and at least 1 year of experience with homelessness or addiction; Addiction certification preferred.

Qualifications

• High School Diploma or equivalent required; addiction certification preferred.
• Minimum 1 year of experience with homelessness or addiction; ministerial counseling preferred.

Responsibilities

• Develop initial individual case plans with residents.
• Assist residents with paperwork for goals, referrals, and documentation.
• Set goals in areas like ID, employment, education, addiction treatment, spirituality, finances, legal compliance, and housing.
• Prepare and maintain SOAP notes; record progress weekly.
• Maintain a weekly calendar of case management appointments.
• Meet residents weekly to assess progress and stability.
• Refer residents to community agencies aligned with their case plans.
• Provide life skills classes with volunteers.
• Ensure living areas comply with shelter guidelines.
• Discharge residents accurately when appropriate.
• Presents the Gospel message and prays with residents when opportunities arise.
