Sheppard Pratt in Maryland is seeking a Full-Time, on-site Case Management - Unlicensed role within the Segue Residential Transition Program. You will assess and monitor client needs, develop service plans, and connect clients to community resources to support treatment goals.
Responsibilities include enrollment assistance, coordinating appointments, medication management plans, obtaining entitlements, documenting thoroughly, and coordinating transitions to discharge.

The Segue Residential Transition Program assists with helping individuals transition out of state psychiatric hospitals. Segue clients are clinically stable and ready to leave the hospital. The services provided assist clients with learning how to manage their illnesses and develop independent living skills.
