The Advocacy & Resource Center is seeking a dedicated and organized Program Coordinator to join our team. This full-time role develops, maintains, and coordinates person-centered habilitation plans to support individuals receiving services and promote growth.

Responsibilities include meeting with individuals to develop goals, updating plans, ensuring alignment with needs, documenting records, and coordinating with staff and providers.

Competenze

• Bachelor’s Degree in Psychology, Human Services, or a related field is required.
• Minimum one year of experience working with individuals with intellectual or developmental disabilities.
• Strong written communication and organizational skills are essential.
• Ability to work independently and manage multiple priorities.
• Clean, valid New York State Driver’s License is preferred.

• Meeting with individuals receiving services to support goal development and promote personal growth and independence.
• Developing new habilitation plans and updating existing plans in accordance with agency standards and regulatory requirements.
• Ensuring plans are accurately implemented and aligned with individual needs and outcomes.
