Job summary

Hill Country Mental Health and Developmental Disabilities Centers is seeking a Service Coordinator to assess needs, advocate for individuals, and develop Person Directed Plans. You will collaborate with providers to support outcomes and provide skills training, crisis prevention, and documentation with HIPAA-compliant care.

You will schedule meetings, ensure compliance with state and company policies, and respond to crises with guidance and support.

Qualifications

• Bachelor's or advanced degree from an accredited college or university.
• Associate degree in social, behavioral, human service, or health-related field.
• High school diploma with two years of paid/unpaid experience with individuals with intellectual or developmental disabilities.
• Minimum 1 year of experience working with individuals with developmental disabilities.

Responsibilities

• Assess needs and collaborate to develop and execute life management and Person Directed Plans.
• Provide relevant skills training to improve daily life.
• Collaborate with providers and agencies to be a liaison for care and outcomes.
• Write and document accurately with proper grammar for clear records.
• Schedule and conduct meetings following state and company guidelines.
• Maintain compliance with legal requirements and policies.
• Respond to crisis interventions with appropriate supervision.
• Meet with individuals regularly to track progress and ensure compliance.
• Guard confidentiality and comply with HIPAA regulations.
