Hope Services, LLC is seeking a full-time leadership professional to supervise the Care Management Team, oversee clinical and administrative operations, and promote integrated care. The role oversees up to 8 care managers and 3 extenders, drives staff development, and ensures quality service delivery and compliance with program requirements.

Qualifications include licensure (LCSW, LCMHC, LPA, LMFT, or RN) and a minimum of 3 years in care management or related fields; experience with co-occurring

Competenze

• Provisionally or fully licensed as an LCSW, LCMHC, LPA, LMFT, or RN.
• Minimum 3 years of experience in care management, case management, or care coordination.
• Experience with co-occurring Behavioral Health and I/DD needs preferred.

Mansioni

• Provide supervision to a care management team of up to 8 Care Managers and 3 Extenders.
• Lead staff recruitment, onboarding, training, performance management, and discipline.
• Ensure quality service delivery and compliance through case conferences, record audits, and performance observations.
• Participate in home visits and Child and Family Team meetings.
• Ensure timely and accurate documentation of activities per policy and program requirements.
