CommonBond Communities is seeking a Service Coordinator II to provide comprehensive, person-centered coordination for residents at Crown Ridge and Boulevard Gardens in Minnetonka. You will conduct strengths-based assessments, develop asset-based service plans, and respond to crises with appropriate referrals and warm handoffs.

Qualifications

• Minimum 2 years demonstrated experience working within diverse communities, including people of various ages, genders, and backgrounds.
• Excellent interpersonal, verbal, and written communication skills.
• Ability to network and develop community partnerships.

Responsabilités

• Conduct strengths-based assessments and develop resident-centered service plans.
• Respond to resident crises independently with consultation as needed; coordinate warm handoffs to clinical or emergency services.
• Manage a defined caseload of permanent supportive housing households with proactive outreach and engagement.
• Collaborate with third-party property management to address lease violations and coach residents back to good standing.
• Coordinate multi-party case conferences for complex tenancy issues.
• Inform residents of home base site information and programming through a monthly newsletter.
• Involve residents in activities and programs and empower them to lead within their community.
