Rehab Resources seeks a Service Coordinator/Early Interventionist to support families with infants and toddlers in Birth to Three. The role centers on coordinating services, enabling access to supports, and working with a multidisciplinary team to promote each child’s development in natural environments such as home or community settings.

Candidates should have at least one year of experience with families of children with special needs and may hold degrees in early childhood, OT/COTA, PT/PTA,

As a service coordinator, this role serves as the primary point of contact for families, ensuring access to appropriate services and supports, and facilitating communication among service providers to promote the child's development and well-being. In this dual role, they may work closely with families, caregivers, and a multidisciplinary team in the evaluation process and create and implement individualized educational plans that promote the developmental progress of children from birth to three years old. This role occasionally provide specialized instruction or direct therapy services according to degree held and team need.
