Job summary

Rehab Resources in Wisconsin is seeking a Service Coordinator/Early Interventionist to support infants and toddlers and their families within the Birth to Three program. You will coordinate services, communicate with a multidisciplinary team, and promote development through natural environments.

The role may include specialized instruction or therapy based on your training. You will work closely with families, caregivers, and providers to develop and implement individualized plans that advance

Qualifications

• Service coordinators must have at least one year of experience working with families with special needs.
• Candidates may have a degree in early childhood, OT/COTA, PT/PTA, social work, human services.
• Other candidates may apply with consideration for other qualifications such as parent facilitator.

Responsibilities

• Serve as the primary point of contact for families and coordinate services.
• Collaborate with multidisciplinary teams to support child development.
• Create and implement individualized educational plans for birth to three.
• Deliver services in natural environments such as home or community.
