Job summary

San Gabriel/Pomona Regional Center in Los Angeles County seeks a Service Coordinator to provide comprehensive case management for adults with developmental disabilities, coordinating medical, behavioral health, educational, vocational, and community resources.

Under the Adult Services I Manager, you will develop individualized IPPs, document progress, and facilitate meetings with families, vendors, and agencies, delivering supports in office and in the community.

Qualifications

• Bachelor's degree in social work, psychology, human development, sociology, public health nursing, or a related field.
• At least one year of experience working with individuals with developmental disabilities or a related field.
• Master's degree can substitute for the required experience.
• Bilingual in Cantonese is preferred.

Responsibilities

• Consult with the Manager, Client Services Staff, liaison, and specialist staff to coordinate services for individuals served.
• Explain available resources and services to individuals and families, ensuring understanding of options.
• Develop and implement individualized Program Plans (IPP) tailored to each individual's needs.
• Write inter-disciplinary notes and document service plans, meetings, and progress accurately and timely.
• Document interim and annual reports within mandated timelines.
• Initiate purchase of service (POS) through the Regional Center when eligible.
• Present cases at specialized committees as appropriate.
