Job summary

San Gabriel/Pomona Regional Center is seeking a Service Coordinator to provide comprehensive case management for adults served, collaborating with families and community resources to ensure high-quality service support. The role emphasizes compassionate coordination across medical, behavioral, educational, vocational, and community services.

Requires a Bachelor's in a related field with at least one year of relevant experience; a Master’s may substitute. Spanish bilingual is preferred.

Qualifications

• Bachelor's degree in social work, psychology, human development, sociology, public health nursing, or a related field.
• At least one year of experience with individuals with developmental disabilities.
• Master's degree can substitute for experience.
• Bilingual in Spanish preferred.

Responsibilities

• Consult with the Manager, Client Services Staff, liaison, and specialist staff to coordinate services for individuals served.
• Explain available resources and services to individuals and families.
• Consult with various teams regarding the needs of the individuals served.
• Conduct in-person meetings with individuals served, families, vendors, and relevant agencies for service planning.
• Develop and implement individual Program Plans (IPP).
• Write inter-disciplinary notes and document service plans, meetings, and progress.
• Document interim and annual reports within mandated timelines.
• Initiate purchase of service (POS) through the Regional Center for clients ineligible for other funding.
• Present cases at specialized committees as appropriate.
• Perform additional duties supporting departmental and organizational goals.
