Job summary

The successful candidate will coordinate assessments, attend education/transition meetings, ensure Medicaid waiver compliance, and maintain thorough records while operating in a hybrid work setting across office, home, and field visits.

Qualifications

• Bachelor’s degree from an accredited college or university and one year of experience in relevant fields.
• Master’s degree in social work, psychology, public health nursing, teaching or related discipline.
• Bilingual preferred but not required.

Responsibilities

• Coordinate case management services for consumers and families.
• Develop and implement IPPs, track progress, and conduct annual reviews.
• Attend IEP/ITP/IHC meetings and prepare documentation as needed.
• Utilize community resources to support consumers and families.
• Maintain timely documentation and ensure Medicaid Waiver standards are met.
• Coordinate assessments across medical, psychological, educational and vocational domains.
