Qualifications

• At least 2 years of health care experience with LTSS exposure.
• 1 year in care management, or equivalent medical/behavioral health setting.
• Licensed/certified as required by state contract or regulation.
• Bachelor's degree may be required in some states.
• Valid, unrestricted driver's license and reliable transportation.
• Knowledge of community resources.
• Ability to work with diverse populations and settings.
• Independent work style with minimal supervision.
• Strong communication skills (verbal & written).
• Proficiency with Office software and online portals.
• Experience with substance use disorders preferred in some states.

Responsibilities

• Completes comprehensive member assessments within regulated timelines, including in-person home visits.
• Facilitates waiver enrollment and disenrollment processes.
• Develops and implements care plans with members and support network.
• Monitors care plan effectiveness and documents interventions.
• Promotes integration of LTSS and behavioral health services.
• Assesses medical necessity and authorizes waiver services.
• Evaluates benefits and funding sources.
• Facilitates ICT meetings for approval or denial of services.
• Uses motivational interviewing to educate and motivate change.
• Assesses barriers to care and coordinates psycho/social, financial, and medical support.
• Identifies critical incidents and develops prevention plans.
• Collaborates with licensed care managers as needed.
• Travel 25-40% locally as required by state/contract.
