Job summary

Washington County Mental Health Services (WCMHS) is seeking a compassionate Bridge Program Service Coordinator to support individuals with developmental disabilities and autism in home and community settings.

You will coordinate person-centered services, supervise direct support staff, deliver some direct care when needed, and manage essential administrative tasks while partnering with families and community partners to achieve meaningful outcomes.

Qualifications

  • High school completion or equivalent; BA preferred.
  • Three or more years’ experience providing services to individuals with developmental and intellectual disabilities and autism.
  • Knowledge of Behavioral Support Plans, Communication Plans, and Individual Support Agreements for individuals with intellectual and developmental disabilities and autism.
  • Ability to collaborate and work productively with internal and community partners.
  • Ability to document work/services provided into an electronic records system with a high level of accuracy and timely completion.
  • Team leadership experience strongly preferred.
  • Valid driver’s license, an acceptable driving record, and consistent, regular access to a safe, reliable, insured vehicle required.
  • Ability to provide services and complete treatment notes in English.

This role supports individuals with developmental disabilities and their families, coordinates person-centered services, and ensures care aligns with each person’s unique goals.
