Access Case ManagerWCMHS is seeking an Access Case Manager to join a team dedicated to helping children and families access the care, support, and community they need to thrive. Access Case Managers provide critical service coordination and brief therapeutic support for children and caregivers navigating complex emotional, behavioral, and mental health challenges. Working closely with families, schools, healthcare providers, and community partners, Access Case Managers play a key role in strengthening support systems, improving access to services, and helping young people build stability, resilience, and long-term success.Summary of DutiesProvide client care, support, and specific case coordination services to children, youth, and their caregivers with direct supervision for a caseload of 15-30+ clientsDevelop and implement plans of care for a moderately complex/acute caseloadComplete basic client referrals via case presentation to the CCRC committee for personal caseloadComplete advanced client intake assessments, develop and implement treatment plan goals for a moderately complex/acute case loadUse advanced service/ treatment planning skills to maximize client engagement and actively manage caseload rotationDevelop and implement therapeutic group opportunities for children, youth and/or caregiversProvide advanced support and case coordination to foster care providers, adoptive parents, kin caregivers and / or biological parentsWork directly with client and their care team to expand their access to and skillful utilization of their community support networks and coordinate referrals to longer-term servicesEstablish and maintain collaborative working relationships with external agencies, including schools, hospitals, and other designated/social service agenciesEngage internal/ external service providers through case reviews, referral management, clinical consultation, care transitionsCompensation and BenefitsWCMHS provides a comprehensive benefits package including health, dental, vision, retirement, and a generous paid time off package including flexible holidays and 12 vacation days in year one.The starting pay range for this position is $24.87 - $25.88 per hourWorkplace ConditionBachelor’s or master’s degree in psychology, social work, or counseling1 - 3 years of experience working within mental health with specific experience with children, youth and caregivers requiredExperience coordinating between multiple services, programs, or agencies or equivalent skills to manage the communication and organization process of helping clients navigate a system of careDemonstrate a basic understanding of trauma informed care practicesValid driver’s license, excellent driving record, and access to a safe, reliable, insured vehicleThe ability to deliver services and complete treatment notes in EnglishWorkplace ConditionWork takes place in a school and community settings. Frequent transportation of clients in one's own vehicle.
