Job summary

Washington County Mental Health Services Inc. seeks a Case Manager to provide wellness coaching, care coordination, and resource navigation for children, youth, and families.

You will connect individuals with essential services, conduct community-based assessments, and support treatment plans across healthcare, education, and community systems. Responsibilities include identifying needs, making referrals, coordinating with a youth team, and supporting school and home-based interventions.

Qualifications

  • Bachelor's degree in related field.
  • Experience providing support to parents on childhood issues including development, behavior, and attachment.
  • Experience interfacing with community partners.
  • Ability to work cooperatively in a diverse team.
  • Adhere to confidentiality policies.
  • Valid driver's license and access to a reliable vehicle required.

Responsibilities

  • Identify youth and family needs, strengths and goals while addressing barriers to accessing services.
  • Make referrals and follow up on services.
  • Support communication between providers and the youth's team.
  • Assist youth with goal-setting, organization, problem-solving, and self-advocacy.
  • Provide support in schools, homes, and community settings.
  • Document services, referrals, coordination, and progress.
  • Collaborate with clinicians and CYFS staff.

Case ManagerAs a member of the team it is your opportunity to play a vital role in providing wellness coaching, care coordination and support navigating services to children, youth and families. You will help connect individuals to essential resources and care, making a meaningful impact everyday. You will deliver ongoing community based assessment, treatment and supports for children and youth experiencing or at-risk for severe emotional disturbance assisting as they and their families traverse healthcare, community systems and education systems.Summary of DutiesIdentify youth and family needs, strengths and goals while simultaneously addressing barriers to accessing servicesMake referrals and follow up on servicesSupport communication between providers and members of the youth’s teamSupport youth with goal-setting, organization, problem-solving and self-advocacyProvide support in schools, homes, and community settingsDocument services, referrals, coordination, and progressCollaborate with clinicians and other CYFS staffCompensation and BenefitsWCMHS provides a comprehensive benefits package including health, dental, vision, retirement, and a generous paid time off package including flexible holidays and 12 vacation days in year one.The starting pay rate for this position is $24.87 per hour.Summary of QualificationsBachelor's degree in related fieldExperience with providing support to parents around a myriad of childhood issues including but not limited to, development, behavioral management, and attachmentExperience interfacing with community partnersAbility to work cooperatively and responsibly as a member of a team from diverse backgroundsAbide by confidentiality policiesDue to the rural nature of our catchment area, a valid driver’s license, excellent driving record and access to a safe, reliable, insured vehicle is requiredWorkplace ConditionsWork takes place in a variety of office and community settings. Frequent travel between sites over the course of a week.
