Care Delivery CoordinatorWashington County Mental Health Services is seeking Care Deliver Coordinators. We are hiring multiple people for this position. This role is responsible for keeping outpatient services organized, accessible, and on-time across four clinic sites and telehealth platforms. Utilizing the MyAvatar database, the Coordinators manage daily site operations, access to care timelines, and cross-team handoffs.By reducing delays and streamlining access pipelines, this role ensures timelier care across therapy, psychiatry, case management, and peer services. By focusing on access-to-care standards, the Coordinators help the agency better serve our community when they are at their greatest need for care.Summary of DutiesAccess & Pipeline Management: Support access-to-care timelines (such as time-to-first appointment and post-discharge follow-ups), monitor referral and intake pipelines for constraints, and propose operational solutions to leadershipProvider Schedule & Template Administration: Build and maintain multi-site, multi-discipline, and telehealth provider schedules and templates in MyAvatar across individual, group, nursing, psychiatric, and care coordination service linesSchedule Deviation & Cancellation Resolution: Manage daily schedule adjustments, fill cancellation openings, place holds, approve overbooks within policy, and resolve schedule errors to prevent disruptions in client carePanel & Reassignment Tracking: Maintain client panel assignments and trackers in MyAvatar, execute reassignments based on provider leave or capacity, and flag uneven client loads or risks to the CCBHC ManagerCross-Team Workflow & Handoff Coordination: Standardize non-clinical handoff steps among front desk, clinical, and care management teams, trigger outreach workflows, and facilitate daily huddles and weekly access meetingsData Integrity & Operational Reporting: Produce routine performance reports—covering access timeliness, referral outcomes, no-show trends, and scheduling quality—while logging operational disruptions and maintaining data accuracyCompliance & Operational Execution: Handle protected health information in accordance with HIPAA and 42 CFR Part 2 standards, adhere to designated compliance pathways, and participate in rotating site coverage across four locationsCompensation and BenefitsWCMHS offers comprehensive health, vision, and dental benefits and a retirement plan with a generous employer match of contributions. Our industry-leading paid time off package includes 12.5 paid holidays that can be used flexibly, and 12 days each of sick and vacation time accrued in the first year of employment.The starting pay range for this position is $30.44-$31.68 per hour.Summary of QualificationsEducation: High School Diploma/GED required; Associate or Bachelor’s degree in healthcare administration, business, public health, or a related field (or equivalent experience) preferredBackground in healthcare or behavioral health operations, scheduling, or clinic coordination preferredProficiency in multi-site/multi-discipline scheduling and standard office software (email, spreadsheets, calendars)Strong operational problem-solving under time pressure, meticulous data quality and attention to detail, and knowledge of privacy rules (HIPAA and 42 CFR Part 2)Clear, assertive communication without direct authorityWorkplace ConditionsWork occurs in office settings. Extended computer use while sitting or standing.
