National Church Residences seeks Service Coordinators to support seniors at Colman Court in Cleveland. This part-time role (20 hours weekly) offers competitive pay and a comprehensive benefits package. Schedule is Monday-Friday with no evenings or weekends, located at 2065 W.

65th St., Cleveland, OH 44102. Responsibilities include coordinating resident programs, linking residents with community services, and advocating for needs while maintaining high-quality customer service.

Formación

• Two or more years of experience delivering social services to elderly and/or family populations preferred.
• Working knowledge of supportive services and local community resources.
• Demonstrated ability to advocate, organize, problem-solve, and deliver measurable results for residents.

Responsabilidades

• Advocate for residents and organize services to meet needs.
• Coordinate programs and activities for residents.
• Liaise with community agencies and negotiate discounts with low-cost providers.
• Engage residents to identify needs and refer to community resources.
• Develop a Resource Directory of state/local service providers.
• Sponsor educational events on health care, support services, life skills, and referrals.
• Provide quality customer service and address residents' concerns and needs.
• Assist residents in accessing housing services, meals, transportation, and daily supports.
• Monitor feedback from residents and families and relay to appropriate channels.
• Manage internal and external relationships with effective communication.
