Job summary

National Church Residences is seeking a part-time Service Coordinator I to assist Pleasant Valley Colony residents in Plain City, OH. The role focuses on coordinating services to help participants age in place, building partnerships with community providers, and documenting outcomes.

The position requires a high school diploma, two+ years in social services, and strong communication skills. Travel and outreach may be involved to connect residents with needed services and supports.

Qualifications

• High School Diploma or GED required.
• Bachelor’s degree in social work or related field preferred.
• Two+ years of social service experience with elderly or disabled populations.
• Ability to advocate, organize, problem-solve, and deliver results.
• Strong communication and interpersonal skills.
• Working knowledge of Windows, Microsoft Office Suite.

Responsibilities

• Coordinate programs and activities for eligible individuals.
• Liaise with community agencies and develop a Resource Directory.
• Conduct participant assessments and document outcomes.
• Provide quality customer service and age in place support.
• Maintain documentation and care plans and follow up on referrals.
• Travel and outreach as needed to support program goals.
