National Church Residences is seeking Service Coordinators to support Colman Court in Cleveland. This part-time role offers 20 hours per week, Monday–Friday, with no evenings or weekends, and competitive pay of $21.67 per hour.

Qualifications

• Bachelor’s degree in social work or related field preferred.
• Two+ years of experience delivering social services to elderly/family populations preferred.
• Working knowledge of supportive services and local community resources.
• Ability to advocate, organize, problem-solve, and deliver measurable results for residents.

Responsabilités

• Advocate, organize, problem-solve, and deliver results for residents.
• Coordinate programs and activities for residents.
• Liaise with community agencies and network with service providers.
• Identify low-cost providers and negotiate discounts.
• Engage residents to identify needs and make referrals to services.
• Develop a Resource Directory of state/local providers.
• Sponsor educational events on health care, support, life skills, and referrals.
• Provide quality customer service and address residents' concerns.
• Assist residents in accessing community services and transportation.
• Monitor feedback from residents and families and communicate it internally.
• Manage internal/external relationships with strong communication.
