National Church Residences in Plain City, OH is seeking a part-time Service Coordinator I for Pleasant Valley Colony. This role focuses on coordinating housing-based programs, linking residents with community resources, and promoting aging in place with a person-centered approach.

The position reports to the Property Manager and involves outreach, assessments, and documentation to ensure quality service and compliance with program requirements. 20 hours per week with non-exempt status.

Qualificações

• High School Diploma or GED required; bachelor's preferred.
• Two+ years of social service experience with elderly or disabled populations.
• Strong communication and problem-solving abilities; proficient with basic computer tools.

Responsabilidades

• Coordinate programs and activities for eligible individuals on a group basis.
• Serve as liaison to community agencies; document services in the management system.
• Develop and maintain a resource directory of local service providers.
• Organize educational events and link participants to health and support services.
• Conduct participant assessments and collaborate with care teams to achieve goals.
