The role supports housing, employment, education, health, and legal needs; you will document progress, manage IAPs, and coordinate with VA and community partners.

Qualifications

• Masters Degree in Social Work or related field required.
• Two years of experience in social services or veterans programs preferred.
• Experience working with homeless or veteran populations preferred.
• Certification in Substance Abuse Counseling preferred.

Responsabilités

• Assess and coordinate services for veterans across housing, employment, education and health needs.
• Coordinate care with VA personnel and community partners for eligibility documentation and services.
• Maintain confidential client files and document progress in DAP format.
• Complete IAPs with clients and support Case Manager I & II in interventions and discharge planning.
• Conduct home visits to provide case management services and monitor health and welfare of clients.
• Assist with program reporting, data collection, and staff training.
