Qualifications

• Masters Degree in Social Work, Psychology, Counseling, or other related field.
• Two years of experience in social services or related field preferred.
• Experience working with homeless and/or veterans preferred.
• Valid driver’s license required.

Responsibilities

• Assesses veterans referred to the program for housing, employment, education, financial, health, and related needs.
• Coordinates care with VA personnel and community partners for eligibility and services.
• Facilitates client intake and compiles files for program eligibility.
• Develops Individual Action Plans (IAP) with clients and guides staff on interventions.
• Performs crisis intervention and supports staff with client issues.
• Maintains confidential client files and ensures documentation is complete and up to date.
• Documents client progress in Data/Assessment/Plan (DAP) format and updates IAPs monthly.
• Conducts home visits to provide case management and ensure client well-being.
• Assists in data collection, reporting, and program quality activities.
• Provides life skills classes and specialized case management for subpopulations.
