Job summary

U.S.VETS is seeking a Case Manager II – Driver to guide veterans toward independence through comprehensive case management. You will coordinate housing, employment, education, financial, health, and legal services, working with VA personnel and community partners to assemble and verify eligibility documentation.

The role includes home visits and ensuring accurate, timely records. Responsibilities include intake, creating IAPs, updating plans monthly, and delivering life-skills groups.

Qualifications

• Provides case management services to veterans and their families.
• Coordinates care with VA personnel and community partners.
• Maintains confidential client files and HMIS data entry.

Responsibilities

• Assesses veterans for housing, employment, education, finances, and health needs.
• Coordinates care with VA and partners for eligibility and services.
• Facilitates intake, completes Individual Action Plans (IAP), and tracks progress in DAP format.
• Documents updates to IAPs monthly and completes discharge summaries.
• Conducts home visits to ensure health and welfare of clients and supports life skills classes.
• Maintains and submits time sheets, mileage logs, and client expenses.
