Job summary

Qualifications

Responsibilities

• Assesses veterans referred to the program across housing, employment, education, financial, health, and legal needs.
• Coordinates care with VA personnel and community partners for eligibility and services.
• Facilitates client intake and compiles complete client files for eligibility.
• Collaborates with clients to develop Individual Action Plans (IAP).
• Maintains confidential client files in HIPAA-compliant manner; documents and files in proper order.
• Documents progress notes in DAP format and updates IAPs monthly.
• Conducts home visits to provide case management and ensure client well-being.
• Performs data entry into HMIS and other databases; contributes to reports.
• May drive company vehicle for client transportation as needed.
