American Senior Communities LLC is hiring a Senior Transitional Care Coordinator to provide facility information by phone and coordinate care for new residents. The role includes coaching the Care Coordination Center team, handling inquiries, validating insurance coverage, and coordinating clinical assessments and insurance authorization.

The position emphasizes mentorship, collaboration with team members, and ensuring smooth transition processes for residents across services.

Qualifications

• Bachelor’s degree marketing or related field preferred; or three-years equivalent experience in lieu of degree.
• One to three years nursing facility, community relations, sales and/or social service experience required.
• Previous health care admissions/marketing/sales experience preferred.

Responsibilities

• Provide customers with facility-related information via telephonic conversations and assist in coordinating care and services for new residents.
• Coach and mentor the Care Coordination Center team members while handling inquiries and referrals.

Senior Transitional Care Coordinator (STCC) provides customers with facility-related information via telephonic conversations and assists in coordinating care and services for new residents.
