Job summary

American Senior Communities in Indianapolis, IN is hiring a Senior Transitional Care Coordinator (STCC). The role provides facility-related information via telephonic conversations and coordinates care and services for new residents, shaping the intake process and referrals.

You will mentor the Care Coordination Center team, validate insurance coverage across plans, and engage clinicians to initiate clinical assessments and managed insurance authorization.

Qualifications

• Bachelor's degree marketing or related field preferred; or three-years equivalent experience in lieu of degree.
• One to three years nursing facility, community relations, sales and/or social service experience required.
• Previous health care admissions/marketing/sales experience preferred.

Responsibilities

• Provides customers with facility-related information via telephonic conversations and assists in coordinating care and services for new residents.
• Coaches and mentors the Care Coordination Center team members.
• Validating healthcare coverage through all insurance plans, and engaging the appropriate team members to initiate clinical assessments and managed insurance authorization.
