American Senior Communities in Indiana is hiring a Case Manager who serves on the interdisciplinary team and takes planning, responsibility, and accountability for resident care on a designated unit, in collaboration with the Unit Manager (UM) and/or Assistant Director of Nursing Services (ADNS).
You will assess and document resident changes, partner with nursing and facility teams to ensure coordinated care, and uphold the CARE values of Compassion, Accountability, Relationships and Excellence
The Case Manager serves as a member of the interdisciplinary team, assumes planning, responsibility, and
accountability for resident care of a designated unit, in conjunction with the Unit Manager (UM) and/or Assistant
Director of Nursing Services (ADNS), and in accordance with Federal and State regulations and company/facility
policies, procedures and care plans.
