Job summary

Kintegra Health in Charlotte, NC is seeking a Senior TLC Case Manager to drive enrollment of PACE participants and grow enrollee census. You will oversee public awareness, marketing to providers, referral sources, and the community at large to support the program’s goals.

Responsibilities include coordinating education and outreach, liaising with social services for Medicaid applications, and leading promotional campaigns while working with the Director of Community Education and Intake.

Qualifications

• Bachelor’s degree or equivalent experience.
• Minimum of three years of professional level experience in marketing of health care products and services and/or community health services.
• Minimum one year nursing experience with the elderly population.
• NC Driver’s License.
• Must possess excellent communication, interpersonal and conflict resolution skills.
• Knowledgeable of Medicare and Medicaid preferred.
• Experience in independently developing, maintaining, coordinating and carrying out promotional events, internal information programs and public information campaigns.

Responsibilities

• Under the direction of the DCEI, supervises evaluation of potential enrollees to determine needs and eligibility.
• Schedules assessments in home and at PACE Center and completes referral/inquiry home visits to educate about PACE.
• Assists with PACE Medicaid application process and liaises with local Departments of Social Services for timely completion.
• Participates in Interdisciplinary Team (IDT) and related meetings.
• Assists with development and evaluation of overall marketing strategy for recruitment and enrollment of PACE participants.
• Identifies potential public relations problems and advises management on preventative actions.
• Maintains administration and relevant reporting and planning systems.
• Directs production of printed publications and audio-visuals for informational materials.
• Plans and allocates marketing resources within the approved budget.
• Plans and executes STLC community events and in-house events.
