Job summary

Kintegra Health in North Carolina seeks a Home Care Coordinator to develop and implement homecare services for TLC participants in a home-based model. Under supervision of the Center Manager and COO, you will coordinate DME and in-home care services to meet participant needs.

Qualifications include a bachelor’s degree or equivalent, LPN preferred, 1 year experience with frail elderly, NC driver’s license, immunizations up to date, and ability to work in a team with strong communication.

Qualifications

• Bachelor's degree or equivalent experience required; LPN preferred.
• 1 year experience with frail elderly population preferred.
• Current, valid NC driver's license and vehicle.
• Immunizations up to date and medically cleared for direct participant contact.

Responsibilities

• Assess home care needs of frail elderly participants and develop care plans.
• Coordinate 24-hour care delivery and all home care services including DME and supplies.
• Reconcile invoices for personal care service hours and home supplies.
• Coordinate and authorize DME and manage home care supplies and services (e.g., Life Alerts, reminders).
• Interact with nursing facilities during DME coordination and discharge planning.
• Collaborate with Interdisciplinary Team to ensure timely care plan updates and SMART goals.
