Job summary

STLC in Charlotte, NC is seeking a Senior TLC Case Manager to drive ongoing recruitment and enrollment of PACE participants, grow the enrollee census, and lead public awareness efforts of the program to providers, referral sources, and the community.

The role reports to the Director of Community Education and Intake and requires a Bachelor’s degree, 3+ years in healthcare marketing or community health, nursing experience with the elderly, a valid NC driver’s license, and strong communication,

Qualifications

• Bachelor’s degree or equivalent experience.
• Minimum of three years of professional level experience in marketing of health care products and services and/or community health services.
• Minimum one year nursing experience with the elderly population.
• NC Driver’s License.
• Excellent communication, interpersonal and conflict resolution skills; ability to work in a team.
• Knowledge of Medicare and Medicaid preferred.
• Experience in developing, maintaining, coordinating and carrying out promotional events and public information campaigns.

Responsibilities

• Under supervision, supervises evaluation of potential enrollees to determine needs and eligibility.
• Schedules assessments at home and at the PACE Center; conducts education about PACE.
• Assist with the PACE Medicaid application process and liaison with local DSS.
• Participates in IDT and related meetings.
• Develops, implements and evaluates overall marketing strategy for recruitment and retention.
• Identifies public relations issues and advises on preventative actions.
• Maintains administration, reporting and planning systems.
• Directs production of printed publications and multimedia informational materials.
• Plans and manages marketing resources within approved budget.
• Plans and executes STLC community events and internal meetings.
• Builds and maintains community relationships; acts as liaison during enrollment.
