Job summary

Senior TLC Gastonia, NC is seeking a licensed pharmacist to provide oversight and advisement on pharmaceutical treatment for participants. The role reports to the Medical Director and collaborates with care providers and Kintegra Pharmacy staff.

The ideal candidate has NC licensure, a Pharm.D. or BS in Pharmacy, and at least three years of clinical pharmacy experience with geriatric populations. Experience with pharmacy software and FQHC or public health settings is preferred.

Qualifications

• Bachelor of Science in Pharmacy or Pharm.D. degree.
• Three years of clinical pharmacy experience with one year with a frail or elderly population.
• Unrestricted Pharmacy license in the state of North Carolina.
• Ability to learn, teach and research; analytical problem solving; team environment.
• Familiarity with pharmacy software; prior FQHC or public health experience preferred.

Responsibilities

• Clinical therapy management
• Collaborates with physicians, nurses, and other healthcare professionals, providing education, communication and support for healthcare staff, participants, and families.
• Collaborates with Senior TLC Providers in medication review and management during assessments and ongoing as required.
• Facilitates monthly Polypharmacy meetings with Providers.
• Participates in the Senior TLC Falls Committee.
• Co-facilitates Pharmacy Committee with Kintegra Lead Pharmacist.
• Monitors drug utilization patterns for potential fraud and abuse TGH data analysis (including those prescriptions that are e-prescribed).
• Conducts data analysis of pricing data to detect outliers and inappropriate changes in formularies.
• Leads the Part D Risk Assessment process in collaboration with Quality and Compliance.
• Participates in participant care planning and records care plan information per protocols.
• Supports Interdisciplinary Team and promotes unity among the team.
• Supports team members and adheres to the 'do no harm' contract; emphasizes teamwork.
• Supports Senior TLC’s mission, vision, and values.

• Supports the Interdisciplinary Team and promotes unity among the team while interacting with the team, other co-workers, and/or participants.
• Supports team members and functions as a team player while providing that support and adheres to the “do no harm” team contract statement. Emphasizes teamwork and collaboration in all interactions.
