Job summary

Kintegra Health in Shelby, NC seeks a Home Care Coordinator to develop and implement homecare services for Senior TLC participants, coordinating DME and supplies and ensuring care in a home-based model under the Center Manager.

The role requires a Bachelor’s degree or equivalent, 1 year with frail elderly, NC driver’s license, teamwork, strong communication and immunizations; you’ll work with the IDT, RN, PCP and Social Worker to plan care, manage visits, discharge planning and resource

Qualifications

• Bachelor’s degree or equivalent experience; LPN preferred.
• Minimum of 1 year of experience working with frail elderly population.
• Current, valid NC driver’s license and vehicle.
• Must collaborate in a team and possess strong communication, interpersonal and conflict resolution skills.
• Immunizations up to date before direct participant contact.

Responsibilities

• Assess home care needs of frail elderly population and develop care plans.
• Coordinate 24-hour care delivery and personal care services to meet participant needs.
• Reconcile invoices for personal care service hours and home supplies.
• Coordinate and authorize all Durable Medical Equipment (DME) and home supplies.
• Assist with discharge planning from nursing facilities to home.
• Support Interdisciplinary Team and promote teamwork in clinic interactions.
• Communicate with IDT when evaluating potential enrollees.
• Participate in care planning and update care plans per protocols.
• Work with clinic RN and PCP regarding in-home nursing needs.
• Perform acute in-home visits as requested by primary care provider or supervisor.
• Collaborate with Social Worker to provide community resources.
