Job summary

Senior TLC in Charlotte, NC is seeking a Case Manager to oversee recruitment and enrollment ofPACE participants, driving growth and maintaining enrollee census. The role includes leading outreach efforts to providers, referral sources, and the community.

The candidate should have a Bachelor's degree or equivalent experience, at least three years in healthcare marketing or community health services, and one year nursing experience with the elderly. A NC driver’s license is required.

Qualifications

• Education: Bachelor’s degree or equivalent experience.
• Experience: Minimum of three years of professional level experience in marketing of health care products and services and/or community health services.
• Minimum one year nursing experience with the elderly population.

Responsibilities

• Supervises evaluation of potential enrollees to determine needs and eligibility for enrollment.
• Schedules assessments in the home and at the PACE Center and completes referral visits to educate about PACE.
• Assists with Medicaid application processes and coordinates with local Departments of Social Services to ensure timely completion.
