Qualifications

• Master's Degree in Social Work (MSW) from an accredited university.
• Licensed Clinical Social Worker (LCSW) or equivalent licensure preferred.
• Case Manager Certification (CCM) preferred.
• 3+ years’ experience with Medi-Cal population or similar settings.
• Strong analytical and problem-solving abilities.
• Knowledge of LTSS and community resources; Spanish preferred.

Responsibilities

• Outreach to Blue Shield of California Promise Health Plan members.
• Conduct psychosocial assessments and develop care plans.
• Provide intensive interventions and linkages to community resources.
• Promote health equity and support stable community living.
• Coordinate with health teams and document in standard systems.
