Job summary

Blue Shield of California Promise Health Plan seeks a Senior Social Worker to conduct assessments of social drivers of health and provide intensive interventions. You will link members to community resources to remove barriers and help them access and manage medical care, supporting stable living in the community.

The role emphasizes experience with Medi-Cal populations and knowledge of state and federal programs; hybrid virtual work is available with in-office requirements as needed.

Qualifications

• Master's in Social Work from an accredited university required.
• LCSW or equivalent licensure preferred.
• CCM certification preferred.
• 3+ years of Medi-Cal or medical social work experience preferred.

Responsibilities

• Outreach to Promise Health Plan members and assess social drivers of health.
• Provide intensive interventions and linkages to community resources.
• Develop care plans and coordinate with healthcare providers.
• Support members to maintain stability in the community.
