Blue Shield of California Promise Health Plan seeks a Senior Social Worker to assess social drivers of health and connect members with community resources. You will outreach to plan members, conduct psychosocial assessments, develop care plans, and deliver intensive interventions to help members access medical care and stay stable in the community.

Candidates should have MSW, LCSW or equivalent, 3+ years Medi-Cal experience, strong communication and time management skills, and be proficient with

Qualifications

• Requires a Master’s Degree in Social Work (MSW) from an accredited university.
• LCSW or equivalent licensure preferred (LPCC/LMFT).
• 3+ years of experience working with the Medi-Cal population.
• Strong analytical and problem-solving skills.
• Proficient in Microsoft Excel, Outlook, Word and PowerPoint.
• Bilingual Spanish speaking preferred.

Responsabilités

• Outreach to Blue Shield of California Promise Health Plan members and conduct assessments for social drivers of health needs.
• Provide intensive interventions and develop care plans.
• Link members to community resources to reduce social barriers and enable better access to medical care.
• Assist members in managing care and remaining stable in the community.
